This page explains how Medicare rebates may apply to care at Human Movement Co through a GP Chronic Condition Management Plan (GPCCMP).

You may know this pathway by an older name such as EPC (Enhanced Primary Care) or CDM (Chronic Disease Management). It is now officially called a GP Chronic Condition Management Plan, and it may allow eligible patients to access Medicare-subsidised allied health visits through a referral from their GP.

In general, eligibility is determined by your GP. This pathway is typically used when a condition has been present for at least six months and your GP believes allied health care may help as part of your broader management plan. Services Australia states that eligible patients can access up to 5 allied health services per calendar year under a GPCCMP.

What you may need before booking

Before booking, it may help to have:

  • your current GP referral under a GP Chronic Condition Management Plan
  • a basic understanding of how many allied health visits you have available this calendar year
  • any paperwork or referral details your GP has provided
  • any questions you want clarified before you attend

If you are not sure whether your referral is current, valid, or still has visits remaining, it is worth checking with your GP clinic before booking.

Services Australia states that referrals under a GPCCMP are valid for the timeframe stated in the referral, and if no timeframe is stated, they are valid for 18 months from the first service date provided under that referral.
